S-IVB staging figures

On a Saturn V flight, approximately how far downrange and at what
altitude did the S-IV staging occur? (My rough estimate is 1,000
miles downrange and 100 miles high.)

Spot on. On Apollo 11, according to the Apollo 11 Flight Journal

Outboard Engine S-II cutoff occured at 100 nm altitude, 883 nm
downrange.
The next PAO callout (after S-IVb ignition) is at 101 nm altitude,
1000 nm downrange.
