I've heard the roots grow up toward the light sometimes, but the plants
seem to do fine.

Not yet, not that I know of.

Yes, AFAIK.

I have not seen any recent reports on this.

There is a picture of an onion that accidentally sprouted in one of
the space stations, being held by an astronaut, wrapped in a towel to
keep it moist and growing.
