There's not a lot (some, but not a lot) to be gained by releasing a second stage launch vehicle from high altitude but at subsonic speed, and it doesn't take that long to reach the maximum feasible altitude for a subsonic flying vehicle anyway, so that refuelling hardly makes sense. Sylvia. Oh come on, think of all the complications it creates that go against safety and performance.
