Hi,

What is / was the about mode / contingency plan like, if one SRB ignited
OK but the other one failed to ignite?

You die. That scenario is not survivable.

NASA has about 1,500,000 yard/leaf bags stashed inside the VAB - in
the event of this contingency, they will be used to house the remains
of the Shuttle and MLP.

There is no "contingency plan". It would be an unsurvivable disaster. Its
just one of the acknowledged risks of the Shuttle system.

You really think that many would be needed... Tough to bag up vaporized
metal.

It's not just a failure to ignite. I read somewhere the maximum survivable
SRB ignition differential is 100 milliseconds. If one SRB ignites more than
100 mS late, the vehicle will come apart.

I think the igniters are very robust and redundant. It's not like a spark
plug,
more like a mini-rocket that shoots down the interior of the SRB. But
pyrotechnic
devices have failed before, so in theory it could happen.

It's all the enviromental regs - each and every blade of sawgrass and
palmetto that the vaporized metal condenses out on will have to be
triple bagged before shipping to Hanford for burial in concrete vaults
pending the availability of Yucca Mountain.

Google on NASA Standard Initiator for more information about the device
mentioned above.

So I guess it's too naïve to think that the live SRB could somehow be
jettisoned, left to blast off on its own?

The exhaust from the jettisoned SRB would rupture and ignite the ET.
That, and the whole shuttle stack sits on the SRB skirts. A minor point
in light of the first problem, but the remaining stack would fall over.
