"In mid-November, the three main parachutes had to be replaced after
the failure of a connector link left Apollo 15 with only two of its
three parachutes operative at splashdown."
....IIRC, wasn't this determined to have been a burn-thru of one or
more of the tethers by RCS fuel, which compromised the chute
deployment?
