Default Paper copies of NASA documents, where do I find?

Where can I obtain a hard copy of the , Overview of ISS Flight Controller
Ratings on Organizational Risk and Tool Development Survey, February 2004

Starring at a monitor for hours makes my eyes buggy.

Please help

You may find a LCD monitor easier on the eyes.
Alternatively, try changing colours, perhaps green on black.

Sometimes reducing the contrast, and enlarging the font can be helpful.

I find running display/monitor at the highest supported refresh rate
helps. Also, running the resolution at that recommended by the
monitor helps as well. Sometimes you can push the resolution up too
high, and it will seem to work, but the picture isn't nearly as good as
it could be.
