Six docking attempts were required in order to successfully achieve
capture latch

engagement during the transposition and docking phase following
translunar injection.

After docking, the probe and drogue were examined by the crew. Probe
operation appeared

normal and radial marks were noted on the drogue. During all subsequent
operations, the

probe operated properly.

Apollo 15 mission main parachute failure

The failure of one of the three main parachutes of the Apollo 15
spacecraft was

investigated by studying malfunctions in the forward heat shield,
broken riser, and

firing the fuel expelled from the command module reaction control
system. It is concluded

that the most probable cause was the burning of raw fuel being expelled
during the latter

portion of depletion firing. Recommended corrective actions are
included.

ground support equipment designed to provide an acid/base
neutralization of the

propellant in both the liquid and gaseous phases so that it may be
disposed of safely.

During the deactivation operation of the oxidizer from the Apollo 16
command module on 7

May 1972, the scrubber tank of the decontamination unit exploded,
destroying the ground

support equipment unit and damaging the building that housed the
operation. Only minor

injuries were received by the personnel in the area and the command
module was not

damaged. Test results show that the failure was caused by an
insufficient quantity of

neutralizer for the quantity of oxidizer. This insufficiency lead to
exothermic

nitration-type reactions which produced large quantities of gas at a
very high rate and

failed the decontamination tank.

A shuttle development flight test vehicle study

A study is presented that identifies the potential performance
capability of the

production shuttle orbiter for powered flight tests using several
propulsion systems

following vertical takeoff and using J-2 rocket engines following air
launch. Of the

approaches considered, the air-launched orbiter equipped with J-2
rocket engines appeared

to have the highest potential for early shuttle development flights.
With this approach,

mach 4 appeared attainable using 45.5K kg (100K lb) of internal
propellant. Several

issues were identified that require resolution to prove feasibility.

Gemini was the intermediate manned space flight program between
America's first steps

into space with Mercury and the manned lunar expeditions of Apollo.
Because of its

position between these two other efforts, Gemini is probably less
remembered. Still, it

more than had its place in man's progress into this new frontier.
Gemini accomplishments

were manyfold. They included many firsts: first astronaut-controlled
maneuvering in

space; first rendezvous in space of one spacecraft with another; first
docking of one

spacecraft with a propulsive stage and use of that stage to transfer
man to high

altitude; first traverse of man into the earth's radiation belts; first
extended manned

flights of a week or more in duration; first extended stays of man
outside his

spacecraft; first controlled reentry and precision landing; and many
more. These

achievements were significant in ways one cannot truly evaluate even
today, but two

things stand out: (1) it was the time when America caught up and
surpassed the Soviet

Union in manned space flight, and (2) these demonstrations of
capability were an absolute

prerequisite to the phenomenal Apollo accomplishments then yet to come.

This edition brings up to date the history of U.S. agencies for space
exploration, the

NACA and NASA, from 1915 through 1990. Early aviation and aeronautics
research are

described, with particular emphasis on the impact of the two world wars

development and the postwar exploitation of those technologies. The
reorganization and

expansion of the NACA into NASA is described in detail as well as
NASA's relationship

with industry, the university system, and international space agencies
such as the ESA.

The dramatic space race of the 1950 and 1960s is recounted through a
detailed histroy of

the Gemini and Apollo programs and followed by a discussion of the many
valuable

social/scientific application of aeronautics technologies, many of
which were realized

through the launching of successful satellite projects. The further
solar system

explorations of the Voyager missions are described, as it the
Challenger tragedy and the

1988 return to space of the Shuttle program. Future plans are outlined
for a

cooperatively funded international space station to foster the ongoing
study of space

science.

Human Exploration of Mars: The Reference Mission of the NASA Mars

The human exploration of Mars will be a complex undertaking. From 1992
through 1994,

personnel representing several NASA field centers met to formulate a
'Reference Mission'

addressing human exploration of Mars. This report summarizes their work
and describes a

plan for the first human missions to Mars, using approaches that are
technically

feasible, have reasonable risks, and have relatively low costs. The
architecture for the

Mars Reference Mission builds on previous work, principally on the work
of the Synthesis

Group and Zubrin's concepts for the use of propellants derived from the
martian

atmosphere. In defining the Reference Mission, choices have been made.
In this report,

the rationale for each choice is documented; however unanticipated
technology advances or

political decisions might change the choices in the future. One
principal use of the

Reference Mission is to lay the basis for comparing different
approaches and criteria in

order to select better ones. The Reference Mission is not implementable
in its present

form. It involves assumptions and projections, and it cannot be
accomplished without

further research, development, and technology demonstrations. It is
also not developed in

the detail necessary for implementation, which would require a
systematic development of

requirements through the system engineering process. The primary
purpose of this report

is to stimulate further thought and development of alternative
approaches.
