Default 2/3 inch High Performance CCD Needed

I am looking for a high quality CCD chip with following specs.

Color : Monochrome
Resolution : CCIR (760 x 576)
Size : 2/3 inch
Temperature : upto 70 deg C
Spectral Response : 400 nm - 1100 nm
Support Chips : Clock Drive and Signal Processing needed

If any one can suggest a manufacturer it would be a great help.
