How did the engineers end up fixing the SRB SEP blast issue causing
camera fogging with STS-112?

They moved the camera so that it's sheltered from the SRB SEP motors by the
LOX feedline housing. The downside is that the camera is now too close to
the orbiter to see the nose cap, but that's not its main purpose anyway.
