Dovetail Plate with 3/4" Wide Dovetail?

Default Dovetail Plate with 3/4" Wide Dovetail?

I'm looking for a dovetail plate that looks like this:

Top view:

| |
----------------------
The circles represent a slot.
Length would be 6-8" and width 2-3"

Side view:

HHH
|

H is a weight and + screw head. I have the weight and screw rod.

End view:

\ / - Dovetail

The plate and dovetail need to be 1/4" thick. Any ideas where I can get
something like this. Of course, Losmandy is a choice. I'll call them again
Monday.
