Looking thru my Newsweek, I also found this quote on the Perspectives
page (Sept. 8, 2003 issue): "'The scene is set for another accident.' Columbia Accident Investigation Board, in a report last week blaming the destruction of the space shuttle Columbia (and the deaths of its seven astronauts) on NASA's soft-on-safety culture" A. This sucks because Newsweek only has this teeny statement plus one other in the CW column about the CAIB report in its issue. Hopefully, they are preparing at least a one-page story on the release of the CAIB report for next week's issue! B. I think this statement is misleading. I didn't find the CAIB report overall to be saying that the odds are good there is going to be another accident (rather it's busy listing ways to improve how things work in the space shuttle program) and I didn't find it's reporting on the NASA culture to be focused mostly on lack-of-safety but on lack-of-communication. What do you think about Newsweek printing this quote? I notice the quote is not attributable to any one person but to the "Columbia Accident Investigation Board" as a whole.
