As far as point (b) goes, often anon remailers are set up in "chains", i.e. the post travels through more than one remailer. Since remailers are not all that reliable - they tend to come and go - very often a message will be sent through more than one chain to increase the odds that it will make it to its ultimate destination. If that's the case here then it's possible that the message was only sent once.
