Default CAIB report: Change management. OK, by the way, get good managers too.

Things needed before return to flight.

A ready to go unmanned launcher, probably in a silo for protection till needed
for emergency launch of supplies to ISS and shuttles.Needs revendous docking
ability for ISS and preferable for shuttle since we dont know what its
condition might be.

Moose or similiar personal reentry packs for each shuttle and ISS crew member.
A real advantage for ISS why unman the station if just one crew member is ill?

Also this could support more ISS crew members since escape limits them to 3
onboard soyuz.

The ability to land a unmanned shuttle in a emergency.If repairs are
questionable. Why toss out a possibly viable orbiter? Or risk lives if not
needed. Unmanned oerations could be used for station support as well.

ISS lifeboat capacity uprated for a crew of 10 for 3 months.

Redesign tires and other shuttle changes to extrend in orbit time with extra
consumables to months at least.

Redesign RCC to make it more robust.

ALL OF THESE are prudntt doable changes to give the system some back up
capacity, while havuing other useful purposes.

Right now the emergemncy launch capacity could be used to keep ISS supplied and
fully staffed.

Not doing these leads to a preventable loss of another vehicle and crew........
